    Registered Nurse

    Summary of Duties

    • Provide direct patient care, teach, and supervise others in aspects of patient/family care, evaluate patient/family needs, plan patient care, and refer patients for other services as needed.
    • Obtain patient medical history from the patient or family member.
    • Perform physical assessments to identify patient needs.
    • Evaluate the patient’s family dynamics, home environment, and condition to determine appropriate services and obtain physician’s orders.
    • Establish, maintain, and/or revise patient plan of care, and inform physician of any changes.
    • Consult and coordinate care with other active caregivers.
    • Carry out other duties that may be assigned.

    Supervisory Responsibilities

    Instruct and supervise licensed practical nurses and aides in specific care needed to meet the goals of care.

    Education and/or Experience

    • Graduate of an approved school of nursing and one year of experience as a registered nurse.
    • Current certificates, licenses, registrations, and CPR certification.

    Licensed Practical Nurse

    Summary of Duties

    • Provide nursing services to patients under the direction of a registered nurse.
    • Educate patients and their families on the aspects of the patient’s care, disease process, and medications.
    • Identify and report to the coordinating nurse and physician health needs of patients and families through the observation of their physical, social, and emotional needs.
    • Contribute to the development, implementation, and evaluation of care plans through conferences with the coordinating nurse.
    • Administer medications as per physician’s orders.
    • Carry out other duties that may be assigned.

    Education and/or Experience

    • Graduate of an approved school of practical nursing and one year experience as a licensed practical nurse.
    • Current certificates, licenses, registrations, and CPR certification.

    Certified Nursing Assistant

    Summary of Duties

    • Measure vital signs such as temperature, respiration, and blood pressure
    • Assist patients with activities of daily living (ADLs)
    • Monitor patients and report any changes
    • Collect samples for testing
    • Monitor food and liquid input/output
    • Provide social support and companionship

    Qualifications

    • Active CNA license or Home Health Aide certification
    • Authorization to work in the United States
    • 6 months previous experience as a CNA/HHA

    Personal Care Assistants

    Summary of Duties

    • Assist patients with personal care needs: bathing, elimination needs, transferring, dressing/undressing, ambulation, and household tasks essential to the patient’s health in accordance with the personal care plan.
    • Assist patients with homemaking, including meal planning and preparation.
    • Shop for groceries and essential household or medical supplies.
    • Accompany patient to medical appointments when approved by the RN Supervisor.
    • May be asked to assist with other duties.
    • Document care daily and submit all required paperwork weekly.

    Certificates/Qualifications

    • Successful completion of a personal care assistant training program or completion of equivalency exam with written proof of passing the exam.
    • Previous experience in a home environment desired.

    Must be mature, emotionally stable, and display a sympathetic attitude toward patients at home with medical problems.
