    Family Room

    The Family Room is a secure, online portal that provides designated family members with information about the care of their loved one. Through the Family Room, family members can, regardless of location, get information on their loved one’s schedule, the caregiving team, medications, invoices, and even specific tasks that were completed during a caregiver’s visit.

    Pennsylvania Veteran’s Registry

    The Pennsylvania Veteran’s Registry is an online application that allows veterans, family members and people who work with veterans to connect with Department of Military and Veterans Affairs (DMVA) to request information related to the valuable state benefits, programs and services offered.  Click here to complete the application and connect with your local DMVA.

    A health care resource for veterans who are disabled, chronically ill, or in need of specialized care, this facility serves 236 veterans and spouses, featuring 196 beds with full nursing care and 40 beds for residents with dementia/Alzheimer’s disease.

    Pennsylvania Department of Human Services Helpline

    Call 1-800-692-7462 for assistance with Medicaid eligibility determinations.

    Community HealthChoices

    For those 21 or older who have both Medicare and Medicaid, or receive long-term supports through Medicaid because they need help with everyday personal tasks, coverage will be provided by Community HealthChoices. This component is overseen by the Department of Human Services’ Office of Long-Term Living and the Department of Aging.

    LIFE

    (Living Independence for the Elderly)

    LIFE is an option that allows older Pennsylvanians to live independently while receiving services and supports that meet the health and personal needs of the individual. Living Independence for the Elderly (LIFE) is a managed care program that provides a comprehensive all-inclusive package of medical and supportive services. The program is known nationally as the Program of All-Inclusive Care for the Elderly (PACE).
